*** Effective Jan 2018, I am only accepting a SELECT few Insurance and EAP providers– Blue Cross Blue Shield, Humana, and Cigna. If your provider is not one that I am accepting, I will be considered as an Out of Network provider. I will gladly provide a Superbill for you to submit to your provider, in order for you to obtain direct reimbursement.
